In some cases, the 'Operation Failed' message is displayed in Outlook because it's not compatible with the file you're trying to attach file in Outlook. The size limit in this error is 20 MB. The email should be able to be sent as long as it is under 20 MB. If the file is larger than this, you must open it in a new window in your computer to ensure that it will be compatible.
